Working closely with the Population Health teams, primary care providers (PCP’s), networked facility partners, and Practice Operations at Privia Health, the Health Advisor will provide care coordination and support for Privia’s at-risk patients. S/he will provide remote support for care management teams. Primary work will include patient outreach to provide coordination services, fill gaps in care, and promote improvements to patient outcomes.  The candidate will have relevant clinical experience, case management experience, educational background or license/certification to support the Care Management team.

Primary Job Duties: 

  • Assist physician practices in identification and completion of quality care goals through direct patient outreach
  • Collaborate effectively with integrated care team to support care coordination for patients
  • Review patient non-compliance lists and conduct telephonic outreach to ensure timely disease specific and wellness visits
  • Motivate patients to be proactive about their health
  • Provide education and assistance with patient self-management goals
  • Assist patients in accessing care at the appropriate source 
  • Make appropriate referrals to internal care team, community resources, home care, and disease management programs
  • Work with patients on their care surrounding: medical, social, family, economical, and behavioral issues
  • Assist in scheduling PCP or specialist appointments
  • Manage a set caseload of patients
  • Engage patient, family, and caregivers to assure a well-coordinated treatment plan is established
  • Perform other duties as assigned

Minimum Qualifications:

  • Long term, skilled nursing facility or home health nursing assistance experience 
  • Certified Nursing Assistant, Certified Medical Assistant preferred
  • Preferred experience with profession-based telephonic or remote communication
  • Proficient with Microsoft Office applications including Word, Excel, and as well as Google Docs 
  • Must comply with HIPAA rules and regulations

Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception and the ability to adjust focus.
