Working closely with the Population Health teams, primary care providers (PCP’s), networked facility partners, and Practice Operations at Privia Health, the Health Advisor will provide care coordination and support for Privia’s at-risk patients. S/he will provide remote support for care management teams. Primary work will include patient outreach to provide coordination services, fill gaps in care, and promote improvements to patient outcomes. The candidate will have relevant clinical experience, case management experience, educational background or license/certification to support the Care Management team.
