Overview of the Role:

The Nurse Care Manager works closely with the population health team, networked facility partners, primary care providers, and practice operations to ensure seamless transitions in patient care. The Nurse Care Manager will provide extensive care coordination for Privia’s highest risk patients.

Primary Job Duties:

  • Manage a robust case load
  • Develop trust and build rapport with a select Privia patient population
  • Establish “call me first” relationship with your patients
  • Engage patients telephonically, in office, in facility, or where needed
  • Act as the liaison and advocate between the PCP and patient
  • Assist patients in accessing care at the appropriate source
  • Have attention to detail for patients: medical, behavioral, social, and economic issue- work to mitigate these barriers
  • Develop relationships with the care coordination teams
  • Understand and utilize the Athena Clarity and Privia Applications to determine high value opportunities
  • Review patient non-compliance issues and conduct outreach to close them
  • Maintain a courteous and helpful manner when working with patients, physicians, and associates
  • Schedule your paneled patients for diagnostic testing and follow up to ensure completion of testing
  • Under direction of the Director, Population Health, work in collaboration with the Manager, Quality and the Health Advisor to achieve Privia Targets for Quality Improvement
  • Participate in special projects and perform other duties as assigned
  • Evaluate and utilize data from EMR to support patients needs
  • Develop and implement shared care plans across disciplines

Minimum Qualifications:

  • BS in Nursing (or equivalent) and at least 3 years of case management or care coordination experience. 
  • Ambulatory Care Management, Home Health, or Long-Term Care experience preferred; Medicare Advantage experience preferred
  • Experience with triage preferred
  • Active unrestricted license in TX or able to obtain (if needed)
  • Must comply with HIPAA rules and regulations

  • Strong computer and EHR skills and expertise 
  • Must be detail oriented and possess varied analytical skills

  • Flexible, as travel may be required
